We appreciate you taking the time to fill out the form as it is important that we keep records of student absences to meet government audit requirements. 

In compliance with government regulations; and to satisfy the school regarding the safety of your child/children, it is that we have a note of explanation regarding the absence.

All absences, early dismissals and lates must be reported to the school office via online form no later than 8:45 AM the day of the absence. This form can also be filled out for past absences in the school year that were forgotten or not reported.

    • By completing this form, both the school office and the students' teacher(s) will be made aware of the absence/late.

  • WRITTEN PARENT PERMISSION from BOTH families is required by the school before food will be given to any child. If you would like the school to give your catered lunch items to another child, please state the name and grade of the child AND ensure both families have sent permission emails to the school office (office@olapoco.ca).
